Enter License/Certification Information

CPSEPortal

1. Under “My Account” choose “My Profile”

2. Under the grid “Licenses/Certifications/Professions” Click Add

2.1 Choose Profession

2.2 Choose Credential Type

NOTE: Certificates are typically for teachers and licenses are for OT, COTA, PT, PTA, SLP, etc..

2.3 If licensed, input license number

NOTE: NY License numbers are 6 digits.

For information or license look up, refer to Office of Professions website: http://www.op.nysed.gov

2.4 Input Effective and/or Expiration Date, if applicable

2.5 Click Save
