NPI is an Organizational NPI
There are two types of NPIs issued by NPPES. One is for individuals and the other is for organizations. The NPI that is listed on the individual's profile is for an organization not an individual.
Typically this happens if an individual creates a corporation. For example, assume an SLP named Susan Johnson. When Susan became an SLP she obtained an NPI for Susan Johnson. Later, Susan creates a corporation named Springfield Speech Services and obtains an NPI for Springfield Speech Services. On the SLP's individual profile, Susan Johnson should be using her original NPI issued to Susan Johnson. When Susan Johnson writes a speech referral or signs a session note, she is doing that as Susan Johnson CCC-SLP, not as Springfield Speech Services.

CPSE Name differs from NPI Registry Name
The name in CPSE Portal is similar but does not exactly match what the NPI Registry has for this individual. Typically this happens when a name is changed based on marital status.
If the name in CPSE Portal is not correct / the most recent, then the individual should update their name in CPSE Portal.

CPSE Name differs from NY License
The name in CPSE Portal is similar but does not exactly match what the NYSED Office of the Professions has for this individual. Typically this happens when a name is changed based on marital status.
