Upload Confirmation of Service Delivery Log
There are two places where a Service Confirmation Log can be viewed by the County.
1. Enrollment Lookup
- Lookup > Enrollment Lookup
- This option can be used by Counties and Service Providers.
- When you scroll to the right there will be a clickable link for “Service Conformation Logs”.
On the Upload Confirmation of Service Delivery Log screen, you will see the child’s Enrollment Info displayed.
In the Upload File section there is an option to:
- Select the Service Month. *This is required
- Browse your computer to select the file you wish to upload. *This is required
- Enter a description for the file you’re uploading. The field can fit up to 250 characters.
- Select the button to Upload the File to attach it to the selected enrollment.
Once the file is uploaded, your screen should look like this. Please note that you can click on the File link to open a copy of the uploaded file.
2. Child Look Up
- Lookup > Child
- This option can be used by Counties.
- When you bring up the child you will need to click “Details”.
- Then you would locate the enrollment and click “Details”.
- This will show you any Service Conformation Logs that have been uploaded for the enrollment you’ve selected.
