Parent / Caregiver Signature Logs

Blank Parent/Caregiver signature log forms from counties will be posted here.

Therapists can print out a pre-filled signature log with Child's information, therapist information, agency information, etc. by going to Caseload Maintenance>My Caseload>

Check off the box next to the Child/Enrollment to print for and press the "generate blank signature logs for selected enrollments".

Choose your options to generate a prefilled log.
