This checklist should serve as guidance when completing the OPRA Enrollment Form to ensure that all of the required fields on the enrollment form have been completed or marked with N/A (if the question does not apply).
If any of the items on this checklist have not been completed or marked as N/A, the OPRA enrollment form will be sent back to the provider along with this checklist. The marked items will need to be included on the enrollment form and returned to eMedNY when completed. The updated OPRA enrollment form should be returned to eMedNY as soon as possible so your agency's billing is not impacted.
