Upload Confirmation of Service Delivery Log
 
There are two places where a Service Confirmation Log can be viewed by the County.

1. Enrollment Lookup
  1. Lookup > Enrollment Lookup
  2. This option can be used by Counties and Service Providers.
  3. When you scroll to the right there will be a clickable link for “Service Confirmation Logs”.

On the Upload Confirmation of Service Delivery Log screen, you will see the child’s Enrollment Info displayed.
If the provider uploaded a form, you will be able to click on the name of the file to download and view it.
 If the provider did not upload a form you will see 'No Service Confirmation logs found'.

 

2. Child Look Up
  1. Lookup > Child
  2. When you look up the child you will need to click “Details”. 
  3. Then you would locate the enrollment and click “Details” located next to the 'Attendances' link.
  4. This will show you any Service Conformation Logs that have been uploaded for the enrollment you’ve selected.
