eSTACs - How to enter a CB placement with a nurse

If a child will attend Center-based classes with a nurse (RN/LPN), a special set of steps need to be taken by both the district and the county. This is because SED categorizes RN and LPN aides as special programs and requires the STAC-812 form.

Please refer to the below steps when entering a CB placement with a nurse.

1) District: 
The district will receive the child's IEP stating that they need a 1:1 RN or LPN. In eSTACs, they will enter the student under the regular program without an aide and submit the placement to the county.

2) County:
Next, the county will send the STAC of the child, with the regular program, to SED for approval.

3) County:
After SED has approved the child for the regular program, the county will then send the completed STAC-812 form to SED.

4) County:
After SED has also approved the STAC-812 form, they will change the program approval for the child from the regular program to the special program with a nurse on EFRT. Once the county catches this development, they will need to inform the district to perform step 5.

5) District:
The district will need to amend the placement and change the program from the regular program to the special program with a nurse. (This is why the special program is available in the dropdown for the district to select but should not be selected in the very beginning.)

6) County:
After the county synchronizes the district's amendment, the student's STAC in Preschool will reflect the special program. At this point, the county needs to mark the STAC as "paper transmission" to prevent it from being sent to SED electronically. Any further amendments and changes to the STAC must be done on paper.
